The Center for Transformative Grief Justice, Inc. is a nonmedical, community-based nonprofit organization with a Type 2 National Provider Identifier (NPI) 1780529867. We are in the process of completing New York State Medicaid enrollment as a Community Based Organization (CBO) provider so that we can participate in Social Care Networks and partner more directly with Medicaid health plans and providers.
Our current services are non-clinical and include trauma-informed case management, supports brokerage, housing and benefits navigation, peer support, and workforce and skills programming for individuals and families impacted by grief, trauma, and systemic inequities.

Founded by Tina White, whose life has been shaped by profound loss, poverty, and the failures of existing systems, the Center for Transformative Grief Justice is a community-rooted response to these challenges.
